Required Forms

Listed below are the required forms for Adult and Youth athletes. These forms can be given to a coach on the athlete's first day of participation, or scanned and emailed to the Program Director.

RRC Liability Waiver

Float Test (required in order to row without a life vest)

Adult Medical Release

USRowing Waiver (not required for members with a USRowing membership)
