Required Forms

Listed below are the required forms for Adult and Youth athletes. These forms can be given to a coach on the athlete's first day of participation, or scanned and emailed to the Program Director.

Float Test (not required for Learn to Row)

Adult Medical Release

Vaccination Attestation or proof of Vaccination (for vaccinated athletes)

Doubles & Pairs Waiver (for unvaccinated athletes who wish to row a 2x/-)

Youth Forms

Youth Medical Release

Vaccination Attestation or proof of Vaccination (for vaccinated athletes)
